
Request for Transcript/Records Form
A signed release and photo ID that includes the student’s signature are required prior to releasing any student’s school record.
Once a student reaches 18 years of age, only the student can authorize release of his/her record (ORC3.22). This means that parents/guardians of students who are 18 or older cannot request or receive their student’s record without written authorization or a Request for Transcript/Records form signed by the student.
Government agencies such as penal institutions, Social Security Administration, and Adult Parole Authority are not assessed a transcript fee. Anyone else must pay the $5.00 fee (money order, cashier’s check or business check) before receiving a student’s record. Do NOT send cash through the mail. Any request paid by cash will not be processed and will be returned to the requestor.
Transcript requests may be made in person during Student Assignment business hours (7:30 a.m. to 4:15 p.m., Monday through Friday), or by mail:
