Child Care Provider
A parent/guardian needing to add, delete or change a child care provider must complete this form at the Student Assignment Office.
Parent/Guardian needs to fill out a Child Care Provider Form with the child care provider’s name, full address including zip code, and phone number. Parent/Guardian must present a photo ID that includes their signature at the time of adding, deleting or changing a child care provider.
Transportation to/from a child care provider is a service provided for students in grades K-8. If the preK-8 student lives 2.0 miles or more from their child care provider, they will be notified by the transportation department in 1-3 school days with the bus stop information.
